When a multi-speciality clinic came to us, they were spending ₹1.2 lakh a month on Meta ads and getting patient leads at ₹2,200 each. Worse, their front desk reported that half of those leads never picked up the phone. Sixty days later, the same budget was producing leads at ₹800, and the show-up rate had doubled. 1. We stopped targeting 'everyone in the city'
The account was running one campaign targeting the entire city, ages 18–65. That sounds like reach; in practice it means Meta spends most of the budget finding the cheapest clicks, not the likeliest patients. We rebuilt around intent signals: people who had engaged with health content, lookalikes built from the clinic's actual patient list (uploaded as a customer audience), and a tight radius around the clinic for high-urgency specialities.

- Radius targeting matched to how far patients actually travel: 5 km for dental, 25 km for orthopaedics

2. The creative answered the question patients actually ask
Healthcare ads fail when they talk about the clinic. Patients don't care about your new machine; they care about their pain, the cost, and whether the doctor is trustworthy. We replaced facility photos with short doctor-on-camera videos answering one specific question each: 'Does a root canal hurt?' 'When does knee pain need surgery?' Cost per lead dropped 40% within two weeks of the creative switch alone.
3. The form filtered instead of collecting
We added two qualifying questions to the instant form: which treatment, and preferred time to visit. Lead volume dipped 15%. Lead quality jumped: the front desk was now calling people who had already told us what they needed. This is the trade most businesses refuse to make, and it's the one that changes the economics.

The cheapest lead is rarely the best lead. Optimise for the patient who shows up, not the click that costs least.
